Mad Science of Houston Waiver

Parental Consent, Release of Liability, and Indemnity Agreement
(Applies to all programs: after-school, camps, workshops, special events, and parties)
By registering my child(ren) and/or contracting services for any Mad Science of Houston program or activity, I acknowledge and agree to the following Terms and Conditions:

1. Program Scope and Participation
This waiver applies to all Mad Science of Houston activities including but not limited to after-school programs, summer camps, workshops, birthday parties, special events, and any custom programming whether hosted at Mad Science of Houston lab, party rooms, or at an off-site/rented facility.
I affirm that I am the legal parent or guardian of the registered child(ren) and that I have full authority to enroll them in Mad Science programming. I understand that participation in Mad Science activities is voluntary and involves hands-on experiments, movement, and group activities.

2. Assumption of Risk
I acknowledge that participation in Mad Science programs involves inherent risks, including but not limited to minor injuries, allergic reactions, physical activity-related issues, or accidental exposure to substances. I voluntarily assume all such risks on behalf of my child(ren).

3. Medical Authorization
In the event of an emergency, I authorize Mad Science of Houston staff to obtain appropriate medical treatment for my child(ren). I understand that I am financially responsible for any resulting medical services or emergency transportation.

4. Release of Liability
I hereby release, waive, and discharge Mad Science of Houston, its employees, contractors, affiliates, facility partners, and representatives from any and all liability, claims, or demands for damages resulting from participation in any program, whether at Mad Science facilities or third-party venues.

5. Indemnification
I agree to indemnify and hold harmless Mad Science of Houston and associated parties from any claim, damage, liability, or expense arising from my child(ren)’s participation, including claims made by third parties.

6. Photo & Media Release
Unless I opt out in writing prior to the start of the program, I grant Mad Science of Houston permission to use photos, video, or audio recordings of my child(ren) for promotional or educational purposes, including social media, websites, and printed materials, without compensation.

7. Agreement Acknowledgment
By registering my child(ren) and/or contracting services for this Mad Science of Houston activity, I agree to all the Terms and Conditions and Waivers as outlined above and on the Mad Science of Houston website. I understand this agreement is binding and applies for the duration of my child(ren)’s participation in Mad Science activities for up to one year from the date of registration.
